La WBRT est souvent utilis\u00e9e car elle est efficace, rapide \u00e0 organiser et g\u00e9n\u00e9ralement bien tol\u00e9r\u00e9e. Cependant, chez certains patients, elle provoque des troubles de la m\u00e9moire et\/ou des troubles cognitifs. Nos travaux pr\u00e9liminaires, utilisant l&rsquo;imagerie par r\u00e9sonance magn\u00e9tique avant traitement chez 46 patients, ont d\u00e9montr\u00e9 que l&rsquo;\u00e2ge et la quantit\u00e9 d&rsquo;hyperintensification de la mati\u00e8re blanche (r\u00e9gions de d\u00e9g\u00e9n\u00e9rescence li\u00e9e \u00e0 l&rsquo;\u00e2ge) dans le cerveau avant la WBRT pr\u00e9disaient de moins bons r\u00e9sultats neurocognitifs apr\u00e8s la WBRT. Nous collaborons avec l&rsquo;<em>Alliance for Clinical Trials in Oncology<\/em>, qui g\u00e8re les donn\u00e9es de deux essais cliniques randomis\u00e9s men\u00e9s \u00e0 bien sur la th\u00e9rapie par ondes radio et la radiochirurgie st\u00e9r\u00e9otaxique. L&rsquo;Alliance a accept\u00e9 de fournir les r\u00e9sultats de l&rsquo;imagerie avant le traitement et des tests neurocognitifs apr\u00e8s le traitement de 417 patients, afin que nous puissions \u00e9tudier la relation entre la quantit\u00e9 d&rsquo;hyperintensification de la substance blanche dans le cerveau avant la WBRT et les r\u00e9sultats neurocognitifs apr\u00e8s la WBRT.<\/p>\n<p>Le fait de savoir quels patients sont les plus susceptibles de pr\u00e9senter des troubles neurocognitifs aidera les cliniciens \u00e0 personnaliser la prise en charge des m\u00e9tastases c\u00e9r\u00e9brales. Les oncologues peuvent conseiller aux patients pr\u00e9sentant un risque \u00e9lev\u00e9 de toxicit\u00e9 neurocognitive de subir une chirurgie, une radiochirurgie st\u00e9r\u00e9otaxique, un traitement syst\u00e9mique ou une observation, au lieu d&rsquo;une WBRT, pour les m\u00e9tastases c\u00e9r\u00e9brales.<\/p>\n<p><strong>Ce que recevoir cette bourse signifie pour moi :<\/strong><\/p>\n<p>Cette subvention de la Fondation canadienne des tumeurs c\u00e9r\u00e9brales nous permettra d&rsquo;\u00e9tendre le travail commenc\u00e9 par BC Cancer, qui sugg\u00e8re que les hyperintensit\u00e9s de la substance blanche sur l&rsquo;IRM de pr\u00e9traitement pr\u00e9disent de mauvais r\u00e9sultats cognitifs apr\u00e8s une radioth\u00e9rapie du cerveau entier pour les m\u00e9tastases c\u00e9r\u00e9brales. Nous serons en mesure d&rsquo;obtenir des IRM pr\u00e9-traitement et des r\u00e9sultats cognitifs post-traitement provenant de deux essais cliniques randomis\u00e9s men\u00e9s par l&rsquo;<em>Alliance for Clinical Trials in Oncology<\/em>. Gr\u00e2ce \u00e0 ces donn\u00e9es, nous pourrons confirmer si les hyperintensit\u00e9s de la substance blanche sur l&rsquo;imagerie des patients avant le traitement pr\u00e9disent de mauvais r\u00e9sultats cognitifs apr\u00e8s la radioth\u00e9rapie du cerveau entier.
